While receiving hospice care, you will receive visits from your personal team of healthcare professionals including nurses, aides, social workers, chaplains, and volunteers. Each has been trained to meet the patient’s physical, mental, emotional, spiritual, and personal care needs. Our hospice team will help with bathing, administering medication, wound care, companionship, and more.
Patients don’t face their terminal illness alone. Their family is facing it, too. Hand In Hand helps families by supplementing their loving care with end-of-life care professionals who can recognize the signs of pain and unmanaged symptoms and provide the medical care needed to treat their loved one without a trip to the hospital.
Our hospice nurses work with families on how to administer medication and our aides will assist with your loved one’s bathing and grooming needs. Chaplains and social workers are also available to help you and your family work through the emotions and stress of this difficult time.
In addition, all medication, medical equipment, and medical supplies related to your terminal illness will be provided at no cost to you or your family.
When your loved one passes away, we’re still here. Our bereavement team will be with your family to provide support throughout the grieving process. We know how much It means for your loved one to have all the care and support available to them.
